Description: This was taken during a complaint inspection about mice in an establishment. I found no evidence of mice themselves or activity, but was then presented with 2 plates (including the one in this photo). It was determined that this was an intentional contamination event. The mice were field mice that had been frozen. They were placed on the plates by an employee during transport from the kitchen to the banquet room.

Description: During an inspection of a well, this is what was found. The cap is only a temporary cap that should not be utilized on a functioning well. Typically this cap is installed after the well is drilled but before the pump is installed. All of the wiring is open and exposed.

Description: Temporary event with free yogurt. Too much product being taken out in the sun without cold holding. A refrigerated truck was onsite, but was not being used to keep the yogurt cold until needed, taking too many pallets out and not going through it quick enough.
